To buy a plan using this eStore, watch this video guide or follow these simple steps:

1. Select the plan type you wish to buy under "Our Products" and click "Buy Now".

2. Select the quantity of plans that you wish to buy and your preferred mode of payment then click "Add To Cart".

3. You will be redirected to view your cart. If you are satisfied with the items on your cart, click "Checkout". If you wish to add more items, please click "Continue Shopping".

4. If it is your first time to use St. Peter Online, you will be asked to create an account before you can continue with your purchase. If you already have an account, you will be asked to log in.

5. Upon signing up or logging in, you will be redirected to view the Life Plan Application Form with your pre-filled personal information. You have to fill in the details for the other requested information and upload your valid ID and electronic copy of your signature before clicking "Proceed".

6. You will be redirected to a preview of your Life Plan Agreement for your confirmation. You will be asked to confirm that you agree to the Terms of the Life Plan Contract and to the Terms of Use and Privacy Policy of St. Peter Online before you can proceed to payment.

7. On the payment page, you can pay for your purchase using your debit or credit card or online bank transfer.

If you are:

• Married & with children: Spouse and/or your children

• Single with children: Children

• Single without children: Parents

• Legal claimants/beneficiaries

You or your family is entitled to the following benefits from your St. Peter Life Plan:

• Memorial Service according to plan type

If the planholder is insurable:

    o Cash Assistance Benefit/Extended Cash Assistance Benefit (for planholders within insurable ages of 18 to 60 years at the time of purchase of the plan and has not reached the age of 65 upon death)

    o Waiver of Installment (for planholders within insurable ages of 18 to 60 years at the time of purchase of the plan)

    o Accidental Death and Dismemberment Benefit (for planholders within insurable ages of 18 to 55 years at the time of purchase of the plan and has not reached the age of 60 upon death)

For Life Plans with Money Back: St. John, St. Paul and St. Francis and St. Andrew, St. Christopher, St. Dorothy:

• Money Back subject to the conditions of Life Plan Contract

It’s easy! You just have to present the following documents to any St. Peter Life Plan, Inc. Office and they will process the claims for you! WE PROTECT AND SAFEGUARD YOUR BEST INTERESTS.

The following are the claims requirements for each specific benefit from your St. Peter Life Plan:

• Requirements in availing of the Memorial Service:

    o Original Life Plan Agreement (LPA) - Planholder’s copy (to be submitted to the branch for death claim purpose if the Planholder is insurable and qualified for death claims.)

    o Original Certificate of Full Payment of Plan (if fully paid)

    o If Assigned, 1 Government-issued ID of Planholder

    o Service Processing Fee of Php500

    o Duly filled-out Memorial Service Call Slip

• Requirements to claim your Cash Assistance Benefit or Extended Cash Assistance Benefit:

    o Original Life Plan Agreement (LPA)

    o Original Certificate of Full Payment of Plan (if fully paid)

    o Registered Death Certificate with seal and issued by the Local Civil Registrar or Philippine Statistics Authority

    o Birth Certificate of Planholder

    o Birth Certificate of Beneficiary (if claimants are children of the Planholder)

    o Marriage Contract of the Beneficiary (if claimant is the spouse and or if Planholder’s daughter is already married)

    o Statement of Claimant

    o Attending Physician's Statement (if plan is still within contestability period and or if cause of death is accident)

    o Photocopy of Valid ID’s of the beneficiary and Planholder (Government-issued IDs)

    o Medical History (if plan is still within contestability period and/or if cause of death is by accident)

• Requirements for Cash Surrender Value (CSV) / Termination Value (TV):

    o Duly accomplished Cash Surrender Value/Termination Value Application Form

    o At least two (2) valid IDs showing the signature of the Planholder (Government-issued IDs)

    o Original Certificate of Full Payment of Plan (if fully paid)

    o Original Life Plan Agreement (LPA)

    o Marriage Contract of Planholder (only if there is a change in the last name of the planholder as indicated in the LPA)

• Requirements for Unrendered Service Benefit (USB):

    o USB Application Form

    o Letter from the beneficiary indicating his/her choice whether to get the payment of the Termination Value or choose the option for the equivalent of one fully paid plan for future memorial service only.

    o Original Life Plan Agreement (LPA)

    o Original Certificate of Full Payment of Plan (if fully paid)

    o Registered Death Certificate with seal and issued by the Local Civil Registrar or Philippine Statistics Authority

    o Birth Certificate of the Planholder

    o Birth Certificate of the Beneficiary (if claimants are children of the Planholder)

    o Marriage Contract of the Beneficiary (if claimant is the spouse and or if Planholder’s daughter is already married)

    o Photocopy of Valid ID’s of the beneficiary and Planholder (Government-issued IDs)

•Requirements for Waiver of Installment:

    o Medical Certification confirming Total and Permanent Disability

    o Medical Records (laboratory results, Medical History, Abstract, etc.) during first confinement and recent medical check up

    o Hospital Bill

    o Police Report (in case accident is the cause of disability)

    o Birth Certificate of the Planholder

    o Personal Investigation of the Branch Manager

    o Photocopy of Valid ID’s of the Planholder (Government-issued IDs)

• Requirements for Dismemberment Claim:

    o Medical Certification confirming the planholder's disability / dismemberment

    o Medical Records (laboratory results, Medical History, Abstract, etc.)

    o Hospital Bill

    o Police Report

    o Birth Certificate of the Planholder

    o Personal Investigation of Branch Manager

    o Photocopy of Valid ID’s of the Planholder (Government-issued IDs)

Processing of documents normally takes about 5 to 10 working days upon filing of the request for claims. The sooner you submit the complete claims requirements, the sooner you will receive your claims benefits.

In order to protect you from unauthorized claims and unforeseen circumstances, all monetary benefits are issued in checks in the name of the proper payees which can be encashed or for deposit to your account in accordance with established banking regulations and practice.

We have branch offices nationwide. You may file your claims at any St. Peter Life Plan Branch Office nearest you.

Retrieval of the deceased from place of death to our chapel as well as the funeral hearse that will be used to transport the deceased from our chapel to the cemetery during the interment is included in all of our life plan packages. There will be applicable charges however, if the destination is in excess of 25 kilometers from the servicing St Peter Chapel.

Purchasing of a St Peter Life Plan does not have any age limit. However, insurance coverage and benefits are only applicable to planholders within the insurable ages of 18 to 60 years (not beyond his/her 60th birthday) at the time of purchase of the plan.

To request for a new Certificate of Full Payment, please submit the following documents to any St Peter Life Plan Branch nearest to you:

• Affidavit of loss

• Two (2) valid IDs

• Replacement fee

To process the correction of your personal information, please submit the following documents to any St Peter Life Plan Branch nearest to you:

• Birth certificate

• Valid ID

• Original copy of signed Life Plan Agreement

• Planholders Identification Card

You may terminate your plan subject to a termination value schedule. For more information and assistance, please visit any St Peter Life Plan branch nearest to your location Mondays thru Saturdays from 8am to 5pm.
